MedMentor exists to ensure every child facing a chronic condition receives every bit of guidance and support they need to thrive.
We began our conceptual phase in early 2017 based upon the persistent spoken need by patients and their families desiring one-to-one relationships to help them navigate through the social and mental/emotional issues associated with a chronic disease. Cole Jackson, founder and ceo, spent two years in development through consultations with patients and families, researchers, medical professionals, non-profit institutions, business leaders, and other entities excellent in their field of community impact. The pilot program was launched in Fall 2017 with a small sample of children with Congenital Heart Disease (CHD) and their mentors, also living with CHD, showing exceptional improvement in pediatric quality of life, and validating the pursuit of a soft launch in January 2019.
To create life-changing relationships between children and adults facing the same chronic illness
That every child lives a full, meaningful life undefined by disease
By partnering with parents, volunteers, and others in the community, we are accountable for each child achieving:
Increased ability to self-manage
Better relationships, greater confidence, and enhanced quality of life
Avoidance of risky behaviors
